Privacy policy
This policy covers information collected through this website. It is separate from the Notice of Privacy Practices you receive as a patient, which governs your protected health information under HIPAA.

What this website collects
This website does not have a contact form. To request an appointment you call the office, or you complete our new patient forms, which are hosted by a third party (see below). We do not ask you to submit health information through this website.
- Usage data. Standard analytics about pages visited, approximate location, device and browser, and how you arrived. We use this to understand which pages help patients, not to identify you.
- Server logs. Our hosting provider records requests, including IP address, as a normal part of serving and securing the site.
- Anything you send us. If you email us, we hold that email. Please do not send detailed health information by email — it is not a secure channel.
Third-party services we link to
Some tasks happen on services we do not operate. When you follow these links you are subject to their privacy policies, not ours.
- New patient forms are hosted by Weave. Information you enter goes to them and then to us.
- Online bill payment is operated by Weave. We do not receive or store your card details.
- Videos are hosted on YouTube. Our pages load a static image, not a YouTube player — nothing is requested from YouTube until you press play, at which point Google may set cookies.
Cookies
This site sets no advertising or remarketing cookies. Analytics may set a cookie to distinguish visits. You can block or delete cookies in your browser settings without losing access to anything on this site.
How we use and share information
We use website information to respond to enquiries, to improve the site, and to keep it secure. We do not sell it, and we do not share it with advertisers.
Your medical records are governed by HIPAA and by our Notice of Privacy Practices, which we provide as a patient of the practice — not by this policy.
Your choices
To ask what we hold about you, to correct it, or to ask us to delete an enquiry, call 801.221.7799 or email receptionist@perioviews.com. Requests about your dental records are handled under HIPAA — ask the front desk and we will walk you through it.
Children
This website is intended for adults. We treat patients of all ages, but appointments for minors are arranged by a parent or guardian, and we do not knowingly collect information from children through this site.
Changes
We will update this page if our practices change, and the date below will change with it.
